[EE] New Employee Onboarding

Instructions: Please download and return completed copies of the 3 forms below. Once completed, you can 1) securely upload the forms with copies of your personal documents using the Dropbox link (a scan or a clear photo of each document taken with your phone is fine) or 2) drop off at the church offices. If no one is present, feel free to leave in the mailbox and let someone know.

Payroll Details: Our payroll is processed every other Wednesday for 26 pay periods per year. Pay Periods close the Saturday before the pay date, and Direct Deposits usually happen overnight on Tuesday/Wednesday.

Form Details

W-4

The W-4 form has a few extra pages with an explainer for how to fill it out and a worksheet to help figure out your deductions, if any.

When returning your completed W-4 you only need to return page 1.

I-9

The I-9 Form includes a list of acceptable documents to verify that you are eligible to work in the US. A Passport is the easiest way to fulfill the requirements. We just need a photo of the page with your picture on it.

Besides a passport, you can submit a Driver’s License & your Social Security Card. Refer to the List of Acceptable Documents on page 3 for more options.

When returning the completed form you only need to return page 1 and photos/scans of your identifying documents.
